FEE STRUCTURE
Initial intake/assessment: $225
Individual hour counseling session: $200
Individual 45 min. session: $155
Individual 30 minute session: $100
Payment: Payment is expected at the time of your visit (unless I am billing your insurance and in that case, just a co-pay will be charged) Payment can be made by cash, check or credit card. VISA, MasterCard, American Express and Discover are accepted. I offer a sliding fee scale for those paying out of pocket. Please ask Kate for details.
Insurance accepted: Aetna, Regence BlueCross BlueShield, and Pacific Source. and OHP Open Card (fee for service)
I am an out-of-network provider for the following; Moda, Providence (PHP), Cigna, HealthNet (MHN), and Optum
Cancellation Policy: I allow one free "no show." If you have a true medical or family emergency, I will not charge you for a missed appointment. In other cases, if you do not show up for your scheduled appointment and have not notified me at least 24 hours in advance, you will be charged $100.00, even if I am billing your insurance since insurance companies do not reimburse for missed appointments.
